You will find various tools and resources below to help determine if your child is developing normally. Please take the time to complete the appropriate screen before your child's check-up and bring the results with you to the appointment.
Developmental Screens
Infant
1) ASQ - complete this screen prior to the 2,4,6,9,12 month check-ups.
2) Lead screen
Toddler
1) ASQ - complete this screen prior to the 15, 18, 24, 30, 36 month check-ups.
2) Lead screen
3) M-CHAT - complete this prior to 18 and 24 month check-ups.
Elementary
1) ASQ - complete this screen prior to the 4 and 5 year check-ups.
2) PSQ - complete this screen prior to every check up after 5.
Preteens and Teens
1) PHQ - complete this screen prior to every check-up.
Other Tools
CDC Milestone Tracker App - an app for your phone to track your child's development.
Postpartum Depression screen - screen for postpartum depression. All moms coming in for newborn and infant visits should take and bring in a printed copy.
Older kids/teens should complete the following screens any time there is a concern.
